According to the General Data Protection Regulation of 27 April 2016, hereinafter referred to as GDPR, we would like to inform you that:
- Your personal data controller is Dagmara Wieczorkiewicz Coaching.
- By using this website you agree to the privacy policy.
- Dagmara Wieczorkiewicz Coaching will be processing the following personal data: name and surname, telephone number, e-mail address and the personal information given during the session in case the coach takes notes.
- Your personal data will be processed for the purposes of contacting you in order to respond to a query and setting up or cancelling the meeting via telephone or e-mail, sending you the notes and homeworks, keeping the record of income (lub bookkeeping activity; w przyszłości także informational and promotional activity).
- The receiver of your personal data will be Dagmara Wieczorkiewicz Coaching.
- Your personal data will be stored for the period of 3 years.
- Under the terms of the GDPR you have the right to:
- access your data,
- rectify your data, should it be inaccurate,
- erase your data, restrict the processing of it as well as the right to data portability – in cases laid down by law,
- object to the processing of your data,
- lodge a complaint with a supervisory authority – President of the Personal Data Protection Office, should you consider that the processing of your data infringes the personal data protection regulations.
The personal data deletion is possible by sending a request via e-mail address bok@coachingdw.pl on the basis of withdrawal of your previous consent.
The controller has the right to process personal data after the service or withdrawal of consent only to the extent that the court proceedings are necessary or the national, Europen Union or international rules oblige us to retain data. Personal data will be stored until the expiry or limitation of claims of the parties that apply to the performance of the service and certain public and legal duties related to it.
- Providing your personal data is voluntary, but necessary to executing the coaching process by Dagmara Wieczorkiewicz. In case of not providing required personal data the execution of the above mentioned in the point 4 goals will not be possible.
- Your personal data will not be disclosed to third parties beyond the territory of the European Economic Area nor to the international organizations.
- Your personal data will not be processed based on automated decision-making, including profiling,which, resultantly, will not be used as rules for taking the decisions.
- All the trademarks (such as Facebook, Instagram), which appear on the website coachingdw.pl, fall within legal protection based on the principles provided for the relevant regulations.
